improve speed

• migrate to fftw (implement Refocus class with its own FFTW instance. #11)
• write in the docs how users can cache FFTW wisdon (don't add appdirs dependency)
• replace np.pad with something faster (in combination with fftw, c-aligned arrays) (-> numexpr?)
